Advanced Directive Workshop

Advance directives are legal documents that allow you to spell out your decisions about end-of-life care ahead of time. They give you a way to tell your wishes to family, friends, and health care professionals and to avoid confusion later on. Anyone over 18 should complete an advance directive to share their wishes at end of life or if they are not at capacity to discuss their wishes. Join staff from Mountain Valley Hospice to learn more about advance directives. Also you will be able to complete your individual advance directives and have them notarized. This workshop is free of charge and lunch will be provided. Please register no later than Thursday, June 21^st.
