The Living-at-Home program provides case management through the Community Alternatives Program for Disabled Adults (CAP-DA). CAP-DA is a Medicaid program that offers a coordinated network of support services facilitating the aged or disabled adult to remain at home for as long as possible or to return home from a stay at a medical facility rather than going into institutional care.
Living-at-Home sends a registered nurse and social worker to complete an in-home evaluation of clients’ abilities, needs and circumstances. Close, ongoing monitoring is then provided by case managers.
The following services are provided on an as-needed basis:
- Assistance with bathing and dressing through in-home aides
- Nursing visits through a home health agency
- Adult day health care at approved centers
- Home mobility aids, including wheelchair ramps
- Telephone alert
- Incontinence supplies
- Nutritional supplements
- Respite care
CAP is available to elderly (65 and older) and disabled (18 and older) adults. The program has specific income requirements, and the cost of services provided must be less than the cost of a nursing facility. A participant must be approved for an intermediate or skilled level of care, and his or her health, safety and well-being must be assured in the home setting.
