Living-at-Home


The Living-at-Home program provides case management through the Community Alternatives Program for Disabled Adults (CAP/DA or CAP/Choice).

CAP/DA and CAP/Choice are Medicaid waiver programs that provide a coordinated network of support services in the home, allowing the aged or disabled adult to remain at home for as long as possible or to return home from a facility, when circumstances allow. The client may elect to participate in traditional CAP/DA or CAP/Choice based on qualifications.

Services Provided

As a standard service, Living-at-Home provides an in-home evaluation of the client’s abilities, needs and circumstances, along with close, ongoing supervision of the client by a case manager/care adviser.

The following services are provided on an as-needed basis:
  • In-home aide or personal assistant services, such as bathing and dressing
  • Adult day health care at approved centers
  • Home mobility aids, including wheelchair ramps
  • Telephone alert
  • Incontinence supplies
  • Nutritional supplements
  • Respite care
  • Home-delivered meals
Eligibility

CAP/DA and CAP/Choice are available to individuals age 65 and older and disabled adults age 18 and older.

To be eligible for either CAP/DA or CAP/Choice, client must
  • Be Medicaid eligible,
  • Need a level of care equivalent to that of a skilled nursing facility, and
  • Have a home situation in which his or her health, safety and well-being can be assured.
Additional requirements must be met for CAP/Choice eligibility.

CAP/DA and CAP/Choice Enrollment
  1. To make a referral, an individual calls Senior Services. The client or responsible person must also call to discuss their interest in the program.
  2. Referral information is processed to determine client eligibility for assessment.
  3. Client information forms are mailed to the client to be completed and returned.
  4. The client must be eligible for Medicaid. At the appropriate time, the client is referred to the Department of Social Services to apply for Medicaid.
  5. Required documentation is completed by the client’s physician and submitted for level of care approval. An in-home assessment is then completed by a nurse and a social worker, and a plan of care is developed.
  6. CAP/DA/Choice services begin after the plan of care is approved.
