The Community Resource Connections of North Carolina (CRC) maintains a coordinated system of information and provides access for all people seeking long-term support and services. With this coordination comes clarity of choices and informed decision making.
A CRC is one of the primary vehicles to modernize long-term care systems by supporting individuals of all disabilities and incomes and helping them make informed, cost-effective choices regarding necessary services. Referring to the CRC may reduce inappropriate or premature institutionalization of a senior.
The CRC is North Carolina's implementation of Aging and Disability Resource Center grants from the Administration on Aging and the Centers for Medicare & Medicaid Services.
Locally, Senior Services is the Lead Agency for the Community Resource Connections of Forsyth County. Trained Help Line Counselors are available to listen to your concerns, complete comprehensive assessments, and share information about the different service providers. Counselors may either refer you to appropriate resources or contact partners within the CRC network on your behalf to begin the referral process. A list of the partnering agencies is below. Counselors often follow up to make sure that you have received the help you need.
In addition to providing phone consultations, our Aging Resource Counselors conduct in-person meetings with seniors who seek information on aging-related matters.
The counselors are also certified by the Seniors’ Health Insurance Information Program and are qualified to assist with Medicare, Medicaid and other insurance questions.
